For any expenses outside of the practice
that exceeds your Member Responsibility Amount
As well as
Preventative Medical Expenses Includes:
Mammograms (after age 40)
Colonoscopy (after age 50)
Flu Vaccines (all ages)
$0 Encounter Cost Utilizing the
PHCS Multi-Plan Network
Further medical expenses, outside of the DPC practice including catastrophic, hospitalizations and surgeries are then 100 percent shared after the Member Responsibility Amount is met.
What is a Member Responsibility Amount (MRA)?
The out of pocket amount the member shares in for their medical expenses for the need.
This is similar to a deductible with insurance.
What is a NEED?
A "Need is the ALL of the medical expenses surrounding a new illness, accident or diagnosis.
All qualified medical expenses after MRA is met is shareable.
Are there Limits?
Up to 1 million yearly / No lifetime limits
Any waiting periods?
All Preventative Services above have a 3 month waiting period,
$0 Encounter fee inside of Network.
Colonoscopy after 6 month waiting period, max sharing $1,500, $0 within PHCS Network,
PCP visits outside of DPC: $100
How can I access care?
Members may use the
PHCS Multi-Plan Network
they can alternately have their providers submit expenses directly to
Alliance for Shared Health
Or self pay and submit to Alliance for Shared Health
Are there any restrictions to sharing?
Alliance for Shared Health has a pre existing condition clause.
A pre existing condition is any condition that existed prior to membership
in which the member had symptoms of or have received treatment for
in the 36 months preceding enrollment.
High blood pressure, high cholesterol and diabetes (type 1 & 2)
are not considered a pre existing condition,
as long as there has not been a hospitalization for the condition
in the past 36 months and able to control through medication or diet.
All pre existing conditions are gradually eligible for sharing in the following manner:
Months 1-12: No sharing,
Months 12-24: Sharing up to $15,000,
Months 24-36:Sharing up to $30,000,
After 36 Months: No Limits
Does Alliance for Health Share in Maternity?
Maternity Needs eligible with a due date of at least 300 days after enrollment
Childhood Immunizations, Flu Vaccines (all ages),
Screening Mammography (after age 40) and Screening Colonoscopies (after age 50)
Tier 1- Medications under $50 - 30 day supply $10
Tier 2-Medications from $51-149 -$20 or 20% Min
Tier 3- Medications $150-$400 -30 day -40% MRA
Members on a high cost medication enrolled into SHARx Patient Advocacy Program at no cost.
SHARx specialized in procuring high cost medications at no or greatly reduced costs.
Members have access to rock bottom pricing with their Shared Health Alliance membership and the ease of using any Quest Lab in the country.
Member receive a self pay rate for imaging services at any Green Imaging Center.
